Everyone has the right to be admitted to Hospice House.

Respite
Respite admission is available for two weeks at a time, for pain and symptom management or for caregiver respite. Respite is offered to those clients with a prognosis of six months or less.

Permanent
Permanent admission is available in the last three months of life.

Steps of Admission:

Registration for admission to Hospice House requires:

• Submission of a referral form including a prognosis provided by your physician
• Hospice clients and/or their designate agree to and sign a No CPR form.
• Palliative Benefits Form – signed by the client and physician
• Recent medical history

Once a referral is received someone from Hospice will contact you to discuss Hospice philosophies of care and the financial aspects of care.
